HEALTH INSURANCE: All students will be charged for Health Insurance. If you DO NOT WANT the Health Insurance, YOU MUST sign a waiver from and return it to the Student Health Office by the Deadline (on back of form). If you do not fill out a waiver and return it by the deadline date, you will be responsible for the payment of the Health Insurance. You cannot get the fee waived after the deadline date. Health Insurance Waivers will be distributed at Orientation.
